A. Municipal / City Permits
1. Business License
a. Barangay Clearance
b. Business Name Registration
c. Lease Contact (if business area is rented)
d. Approved Building Permit
2. Sanitary Permit
a. Approved Business License
b. Approved Building Permit
B. BFAD: For Certificate of Product Registration (CPR)
1. Notarized Letter of Application and Application Form
2. Valid License to Operate (LTO). This must be revalidated for the current year.
3. Product Information
a. List of ingredients in decreasing order of proportion.
For additives with prescribed limit, the amount must be indicated.
b. Certificates from flavor supplier that the flavor components are recognize as safe and suitable for human consumption either by the US Flavor Extract Manufacturers Association or the US Food and Drug Administration (USFDA).
c. Finished product specification (Physico-Chemical and Microbiological).
4. One sample of the product in commercial presentation. For initial / new product application, a proposed artwork of the label may be submitted.
5. Labels and labeling materials used in the product (to be attached in the documents).
6. Certificates of analysis of the finished product. Include the analytical
method used. Submit test results, which are critical to the product. Analysis may be conducted by BFAD’s recognized laboratories (UP, GS, PIPAC, FDC, FNRI and ITDI).
7. Flow diagram of method of manufacturer, packaging and quality control. A packaging certification of suitability for food use must be submitted.
8. Estimated shelf life, parameters used and methods for determining shells life.
9. Justification of label claims(s) which may be supported by:
a. Computation
b. Test Result
c. Rationale
d. Science Study
e. Pertinent Regulations
f. Certificates ( such as Veterinary Quarantine Certificate for meat products, Dioxin free certification, Halal certificates, etc.).
g. Other Supporting Documents
10. Registration Fee
11. Previous Certificate of Product Registration (CPR) for Renewal only.
C. BFAD: License to Operate
1. General Requirements
a. Standard Petition Form, duly notarized
b. 1x1 ID Picture of Applicant (Owner/General Manger)
c. For Corporation /Partnership: Photocopy of Registration with SEC and Articles of Incorporation and Photocopy of DTI Registration of Business Name ( if SEC registered name differs from the Business Name)
d. For Single Proprietorship / Branches: Photocopy of Registration with Department of Trade and Industry
